Kujawiak (ex HMS Oakley, Hunt type II) could not have sank anything on 4/11/42 because she had been sunk herself on 16/6 previous.
Both Hermann and Schleswig-Holstein were lost, but they are different ships:
Hermann was WBS.1 and was lost to ice pressure in Greenland either 17/3 or 17/6/43 (sources are conflicting on the actual date), her tonnage was 284;
Schleswig Holstein was 174 tgr, a local harbor defence craft numbered in the H. series (correct pt# needed), and she was lost 3/8/42 near Alderney, Channekl Islands, in action against British MGBs.
